The Study Leave Completion Form is a mandatory post‑study requirement that staff must submit within two (2) months after the end date of their approved study leave. Its purpose is to validate that the study leave was used as approved and that the learning activity was completed or attended as planned. Submission of this form is required to regularize the study leave. If it is not submitted within the 2‑month timeframe, the approved study leave may be withdrawn and converted to annual leave.


Staff must upload official proof confirming completion or attendance, aligned with the approved study‑leave dates. Acceptable documents include:

  • Certificate of completion
  • Certificate of attendance
  • Official transcript or exam results
  • Official confirmation letter/email from the university or training provider

These documents must clearly indicate:

  • Name of the staff member
  • Name of the course / programme
  • Name of the institution/provider
  • Dates of attendance or assessment


In cases where certificates are issued later (e.g. degree programmes, delayed exam results), staff may submit temporary official proof, such as:

  • Screenshot or PDF of exam results from the university portal
  • Screenshot of attendance from the university portal (if attendance is tracked online)
  • Official email from the institution confirming exam completion or attendance
  • Proof of full attendance aligned with approved dates


Staff are expected to submit the final certificate once available, if requested.


How to submit in Wave:


  • Log-in to Wave
  • Go to Me > Quick Actions (in the left pane) > Document Records. 

  • Click on the button ‘+ Add’. 

  • Select ‘Study Leave Certificate confirming the completion of the training’ from the dropdown.


         

  • Complete the form and upload the supporting documents. 
  • Review the form then click ‘Submit’.


After submission, SDL reviews the completion form and attachments in Wave. If compliant, SDL approves the completion record. The study leave is then considered as fully validated.
